Assist participants to achieve their NDIS goals and build their capacity in their home and in the community (SIL & Social and Community supports). The successful applicant will need to understand the support needs of all participants they work with by adhering to relevant documentation such as care plans, medical plans and weekly schedules. The successful applicant will also be responsible for transporting participants in their own vehicle and maintain accurate records of participant’s km.
